Important Message to Providers from MSDE!
Even if you took the Provider Tuition Survey before, you must verify that all of your vouchers have been entered.
• Go to the Online Survey Verification (also attached) to make sure each voucher from your Scholarship Specific Letter is listed.
• If any voucher is missing, you must enter that voucher into the Provider Tuition Survey.
• If all vouchers are listed, you do not need to take any additional action.
Avoid your scholarships being reissued and parents potentially leaving your program.
